    By completing this application to receive text message alerts from Decatur State Bank, I agree to the following terms and conditions:

    • I understand that one (1) text message will be sent for EACH account that I request a balance for each day and time selected. This can result in multiple text message alerts.
    • I understand that cell phone carrier text message rates may apply and I will not hold Decatur State Bank liable for any text message charges.
    • I understand that when I travel outside of the United States, my carrier may assess additional text message charges, and I am responsible for these charges.
    • I am responsible for notifying Decatur State Bank if my cell phone is lost or stolen.
    • I am responsible for adding, changing and/or deleting my text message alerts by completing the online form provided by Decatur State Bank when there is an account ownership change.
